My brain also has serious problems with "translating" sounds that are at similar volumes.
::nods:: I finally realized with lin's comment above that partly 'cause I hear voices as instruments it's really hard for me to separate out the words from the 'music', and I TOTALLY get what you're saying about background noise...though I don't have personal experience of the physical pain. ::hugs you::
